New Customer Forms

If you are a new customer, the first step to renting is to set you up as a client in our system. If you are renting for your business, please download the BUSINESS APPLICATION, fill out and email or fax back to us. If you are renting for personal use, please download the PERSONAL APPLICATION, fill out and bring in to our office in person on or before your first rental. We need this information in lieu of deposits.

If your business will use us often, simply download the CREDIT APPLICATION. This will enable us to bill you later. Fill it out and email or fax it to us. This process takes 7-10 days to complete, so if your first rental is before that, you will need to pay by check, cash or credit card. If you want to pay with a credit card over the phone, please download our CREDIT CARD FAX FORM. Please fill out and email or fax back to us.

Our ACCEPTANCE OF RESPONSIBILITY form is available to download, but we require you to fill it out only when we specify. It is needed when customers leave our equipment with a third party overnight or over the weekend. Please don’t send this in unless we ask you to.
